Doggy Daycare Assessment

To be used when signing your dog up to join Jollytails Resort doggy daycare for his or her first day.

This form provides your contact information, along with your dog's social history. The form takes approximately 10 minutes to complete, and is required as part of our application/assessment process. Please answer with as much accuracy and detail as possible. The form is delivered to us via email and and we will get back to you.

When you click submit, you will also receive an email invite to join the Jollytails e-mail list. This will keep you up to date on events, information, and exclusive discounts. If you do not wish to subscribe, simply delete or ignore the invitation email and you will not be subscribed.

RELEASE, WAIVER OF LIABILITY AND INDEMNIFICATION

I execute this release, waiver of liability and indemnification (the “Release”) in favor of Jollytails Doggy Daycare Inc., a Nova Scotia Corporation, its directors, officers, employees and agents (collectively, “Jollytails”). I understand and agree that in admitting my dog(s), Jollytails has relied upon my representation that my dog(s) is in good health and has not harmed or shown aggressive behavior towards any person or any other dog for which medical attention was required.

I understand that while Jollytails carefully screens all applications, reviews each dog’s health and vaccination history, and monitors each dog’s behavior in order to provide quality care and service, dog behavior can be unpredictable and certain illnesses not readily apparent.  Accordingly, I hereby release Jollytails from any and all liability, claims and demands of whatever kind of nature which arise or may hereafter arise from my dog’s attendance at Jollytails.  I understand that this release discharges Jollytails from any liability or claim that I may have against Jollytails with respect to any injury, illness or death, either to myself or my dog(s) that may result from me or my dog(s) presence or attendance at Jollytails.

I understand Jollytails outdoor play area is contained by a 6.5 foot fence and my dog will have to full access to this area. If my dog has the ability to jump this barrier, and does so, I will hold Jollytails harmless from any claim.

Furthermore, I will defend, indemnify and hold Jollytails harmless from any claim in relation to me or my dog(s) involvement with Jollytails that may be filed against Jollytails.

I also agree to repair or replace any and all equipment damaged by my dog while my dog is attending Jollytails. Jollytails shall notify me promptly upon discovery of any such damage.

I agree to allow Jollytails to use my Dog(s) name and any images or likeness of my dog(s) taken while he/she is at Jollytails, in any form or format, for use, at any time, in any media, marketing, advertising, illustration, trade or promotional materials.

I agree to pay for all fees, services and products at the time I pick up my Dog(s) after each visit to Jollytails.

No Dog(s) may be abandoned at Jollytails. I agree that I will not neglect to pick up my Dog(s) from Jollytails by the time he/she is scheduled to leave without providing notification. Any dog(s) that is left at Jollytails without any contact, instruction or notification from me, of the ability, willingness or plans to pick him/her up by myself or my personal representative or agent, will be considered abandoned upon the fifth day of such notification failure. I understand that if I abandon my Dog(s) at Jollytails, Jollytails will, by default, become the legal owner and guardian of the Dog(s). Jollytails will, in its sole discretion, will relinquish the Camper to an unrelated shelter of its choice. I FULLY UNDERSTAND AND AGREE THAT IF I ABANDON MY DOG(S) AT JOLLYTAILS, I MAY BE UNABLE TO RETRIEVE POSSESSION OF MY DOG AND WILL HAVE NO RECOURSE AGAINST JOLLYTAILS.

I understand that while Jollytails does its best to provide quality care, it is not necessarily the appropriate environment for every dog. Accordingly, I understand that Jollytails reserves the right to permanently remove a dog from daycare at any time.

I understand that Jollytails is committed to creating a safe, healthy and caring environment for my dog. In the event, my dog needs any medical care or attention while my dog is attending Jollytails; I authorize Jollytails to arrange for such care if they are unable to reach me for whatever reason. I understand that I will be responsible for all costs in connection with my dog, including transportation, veterinary, medical and otherwise. I understand that in the normal running of daycare minor cuts and bruises may occur during normal dog play.

I certify that I have read and understand the rules and regulations set forth on the preceding page and this Release. I agree to abide by the rules and regulations, and accept all terms, conditions, and statements of this Release.
